Case of the Month - March 2003

Signalment:
3 year old, female spayed Pit Bull Terrier with a 3 day history of progressive anorexia and 1 day history of hematuia.
Initial differentials?

Image is a sagittal plane of the urinary bladder

  1. Describe the abnormalities
  2. What are your differentials?
  3. Testing considerations
 

Ultrasound findings:

  1. Generalized hyperechoic bladder wall thickening (approximately 1 cm throughout entire bladder)
  2. The thickening is slightly irregular in the deepest portion of the image with hypoechoic central position surrounded by a hyperechoic rim.

Differentials:

  1. Cystitis
  2. Transitional Cell Carcinoma
  3. Mural Hemorrhage

Testing Considerations:

  1. Urinalysis
  2. Thoracic radiographs
  3. Coagulation panel (physical examination for signs of petechiae, etc.)

Diagnosis:

This dog was found to have very elevated clotting times consistent with Warfarin intoxication. The bladder wall thickening resolved at a rate of 1mm/day with Vitamin K therapy. A repeat US in 1 week was normal.
