Signalment: 10 year old male intact Sheltie

History:  8 week history of polyuria and stranguria

What are your ultrasonographic findings?

What is your diagnosis?

Answer:

Ultrasonographic findings:

  1. Dilation of the left renal pelvis with anechoic fluid is present, allowing visualization of the renal diverticuli.
  2. A large irregular mass of soft tissue opacity is present in the trigone of the urinary bladder.
  3. The right kidney is hyperechoic, has poor corticomedullary distinction, and a mildly dilated renal pelvis.
  4. The ureters are dilated bilaterally.

Ultrasonographic diagnosis:

  1. Acute severe hydronephrosis of the left kidney secondary to obstruction at the level of the trigone of the urinary bladder.
  2. Urinary bladder mass consistent with neoplasia (likely TCC).
  3. Right chronic renal disease (interstitial nephritis), mild hydronephrosis.
  4. Bilateral severe hydroureter.

The findings of the urinary bladder mass explained the stranguria.

The findings of the right kidney explain the polyuria.
